I accept that participation in the Future Stars Tournament is a contact sport and as such, injuries may occur and I assume the responsibility freely. I understand that ABBC Taekwondo, LLC, the organizers, and medical staff are not responsible for the actions of other competitors who may compete against me and that I am responsible for providing the required safety equipment and for my own personal protection at all times. I hereby WAIVE and RELEASE, for myself, my heirs and executors and administrators, any claim that I may make against ABBC Taekwondo, Future Stars Tournament Committee, and volunteers for any injuries I may receive during my participation in this event. I agree that in the case of injury, only basic first aid will be made available on-site and that I am responsible for any and all resulting medical care and expenses. I consent that any photographs of me
participating in this event may be used for publicity without compensation or notification.
I fully understand the competition rules and agree to abide by them. In the event this application is for a minor, I warrant and represent that I am the parent or legal guardian of such applicant and that I waive any and all rights and claims for damages claimed for competitors contained herein. This release, consent, and assumption of risk have important legal consequences. If the competitor is under the age of 18, his/her parent or legal guardian must execute this document.
I also understand that there are NO REFUNDS of any tournament entry fees.
